Check off any of the boxes that may apply to you and bring the immunization checklist to your next appointment with your healthcare professional. The Vaccines411TM immunization brochure is a simple way to get the conversation started about whether your vaccines are up-to-date and for obtaining a healthcare professional's opinion. "Some people might not know how to initiate the discussion about vaccines with their healthcare professional or don't know what questions to ask. This checklist not only helps you determine what vaccines you may need or have received, but can also be used as a record. Just like the Clinic Finder, the Checklist was created to help Canadians stay healthy," explains Francisca Roel, president of FR@SNM and founder of the Vaccines411.ca initiative.
