The Honourable Deb Matthews Opens the Centre for Complex Diabetes Care at North York General Hospital
TORONTO, Nov. 29, 2012 /CNW/ - The Honourable Deb Matthews, Minister of Health and Long-Term Care, attended the opening of the Centre for Complex Diabetes Care (CCDC) at North York General Hospital (NYGH) today.
The CCDC will support and treat patients from within the Central LHIN with complex diabetes needs, such as unmanaged diabetes complications, barriers in accessing health care, and recurrent emergency department visits or hospitalizations.
The CCDC offers shorter-term care with frequent follow-up. As patients meet their goals, and/or clinical targets, they are transitioned to other services for ongoing care.
The CCDC team includes an endocrinologist, nurse practitioners, registered nurses, registered dieticians, social workers, a pharmacist and a chiropodist. The team works directly with each patient's primary care provider to maintain continuity of care and avoid service duplication.
